Step 1: Boat Rental

This form contains basic information required for the rental. It includes name, address, phone number, and some limited drivers license information. This needs only to be filled out once for the group by one individual. This means that whether you are renting one boat or many, only one rental form is required. Only the top portion of this form needs to be completed. We will take care of the bottom half of the form on the day of the rental.

Step 2: Adult Consent

After printing and completing the rental form, every adult (anyone 18 years of age or older) must read and complete the liability waiver form. Please make sure that everyone understands the information provided in the form and that it is filled out completely (don't forget to initial the information at the bottom right of the page.)

Step 3: Minor Consent (if applicable)

Finally, after ensuring that all of the adults have completed the adult waiver forms, every minor (anyone under 18) must also have a form completed. These liability waivers are filled out by a consenting adult or guardian, not by the children. Make sure that each minor has the permission of their parent or guardian before signing them up for a boating trip.
